In last years, diagnostics and treatment of localized prostate cancer were improved. Introduction of a new methods caused rising detection of patients, for whom, due to the age or comorbidities, radical therapy is with relative benefit. Decision, whether single cancer is for concrete patient clinically significant or not, is often complicated. Common prognostic factors are not able to solve this problem. Familiar and hereditary prostate cancer definition leads modern scientific research to detection of genetic dispositions of a single patient to develop a disease. There were detected a number of candidate genes and genomic areas, where alterations...

Prostate cancer is a disease which origin is affected appart from enviromental factors also by genetic factors. Despite extensive research, the ethiopathogenesis of prostate cancer is only slightly explored. According to genetic risk we distinguish between sporadic cancers (mostly prevalent), familial and hereditary cancers. From genetic abnormalities mostly seen in prostate cancer we can name genetic changes in gene for androgen receptor, from other candidate genes there is RNAseL on locus HPC1, genes ELAC2, MSR1, BRCA 1 a 2, fusion genes TMPRSS2+ERG/ETV, KLF6 etc. With the development of molecular biologic methods we have more options to understand...

Ces Urol 2010, 14(3):148-155 | DOI: 10.48095/cccu2010027
Aim:To compare outcome of microsurgical varicocelectomy performed by laparoscopy or open using microscope.Material and methods:Patients (< 18 years old) with unilateral varicocele grade II?III have been randomly assigned to laparoscopic microsurgical repair (Group L) or to subinguinal microscopic repair (Group M). The research protocol contained age, pubertal stage, testicular size, blood level of TH, FSH and testosterone; spermiogram; surgical details and postoperative course were studied. For final evaluation, patients with minimal follow up of one year were selected (57 in group T, 69 in group M).Results:After...

Ces Urol 2010, 14(3):156-163 | DOI: 10.48095/cccu2010028
Aim:The purpose of this research was to retrospectively compare late urinary toxicity of the three-dimensional conformal radiation therapy (3D-CRT) 74 Gy and intensity-modulated radiation therapy (IMRT) 78 Gy for localized prostate cancer.Material a method:A total of 203 patients with Tl-3 prostate cancer were treated with 3D-CRT (n = 91) and IMRT (n = 112). The median follow-up time was 5.2 years and 3.0 years, respectively. The prescription dose was 74 Gy for 3D-CRT and 78 Gy for IMRT. Late urinary toxicity was graded according to the Fox Chase modification of the Radiation Therapy Oncology Group and Late Effects Normal...

Aim:Radiotherapy (RT) in combination with hormonal therapy is standard treatment option in high-risk prostate cancer patients. Although trials of prostate plus whole pelvic RT have not improved prostate cancer outcomes in randomized trials relative to prostate RT alone in the setting of neoadjuvant and concurrent androgen suppression, elective treatment of the pelvic lymph nodes might improve disease-free survival. A dose escalation to the prostate is possible to provide intensity-modulated RT (IMRT) or high-dose rate brachytherapy (HDR BRT) technique. We compared two subgroups of high-risk patients (T3, GS 8-10 or PSA > 20).Material...

Ces Urol 2010, 14(3):173-179 | DOI: 10.48095/cccu2010030
Aim: The verification of microbial findings in the urine of the patients after a cystectomy with urinary tract reconstruction using ileum and determining sensitivity to antimicrobial drugs and ways of the protection. According to results we plan to propose the appropriate prophylaxis. The suitability of subsequent therapy in postoperative period has been consulted with Department of Clinical Microbiology.Material and method: The authors evaluated the cohort of 31 patients after cystectomy with continent derivation of urine in modification Vesica Ileale Padovana (V.I.P.), who underwent surgery on Department of Urology in Hradec...

Ces Urol 2010, 14(3):180-185 | DOI: 10.48095/cccu2010031
Aim: Authors retrospectively evaluate results of extracorporal shock waves therapy (ESWT) in patients with Peyronie's disease after unsuccessful conservative treatment. ESWT was performed on electrohydraulic lithotriptor MEDILIT 7. Time of surveillance was on the average of 13 months.Material and method: We treated group of 18 men aged 29 to 74 years old (on average 56 years old). All patiens were after unsuccessful conservative treatment (he-terogenous group following vitamine therapy, hydro cortisone application, verapamil and ultrasound treatment) - duration of symptoms was more than one year. Plaques were situated on dorsal...

Ces Urol 2010, 14(3):186-196 | DOI: 10.48095/cccu2010032
Aim: Magnetic resonance (MRI) presently represents an eligible diagnostic method for more precise determination of local extension of prostate cancer status using magnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS).In our work we present our own experience with MRI and MRS in the detection of prostate cancer. Methods and results: We present our experience in MRI and MRS in the group of patiens with previously diagnosed prostate cancer (CaP) referred for radical prostatectomy. Histopathologic findings in radical prostatectomy (RP) specimens were compared to MRI results. The average age in the group...
