The authors report 11 (3,47%) patients in the 8-to-79 age-bracket (average age 63,5) treated for an unusual or rare type of primary prostate tumor during the years 1987 - 1996. In 9 patients the rare carcinomas appeared in a mixed form with a regular adenocarcinoma of the prostate. There was a carcinoma arising from large (primary) prostate ducts diagnosed in 4 patients, urotelial carcinoma in 5 patients (1 of them in a pure form), a small-cell carcinoma in 1 patient and a pure rhabdomyosarcoma of the prostate in another 1 patient. 4 patients suffering an incidental adenocarcinoma coming out of the primary ducts have been surviving on hormonal therapy...

Evaluation of the influence of total prostate specific antigen, age and the prostate volume on the free/total prostate specific antigen ratio

The free/total prostate specific antigen ratio (f/t PSA) is significantly lower in patients with carcinoma of the prostate cancer when compared to patients with benign hyperplasia of the prostate. Determination of this ratio increases the specificity of the prostate specific antigen for prostate cancer detection.In a group of 95 patients the authors evaluated the influence of the total prostate specific antigen (PSA) serum concentration, the volume of the prostate and the age of the patient, on the f/t PSA ratio. They demonstrated a statistically significant relation between the PSA levels and the f/t PSA ratio.

The kidney dystopia and skeletal scintiphotography

Scintiphotography of the skeleton was performed on a 71-year old female patient. The scintiphotograph revealed a suspicious focal lesion in the area of the sacro-iliac articulation. Following this finding a static scintiphotography of the kidneys was performed disclosing a sacral kidney dystopia, causing the falsely positive fin-ding in the initial skeletal scan.

A surgical technique sparing the urethra for the following orthotopic replacement of the bladder was elaborated based on studies of the female urethra anatomy. A group of 32 patients with invasive carcinoma of the bladder underwent surgery during the 1993 - 1998 time period. Normal bladder control on a daily basis was experienced by 20 patients. Twelve patients suffer by urinary stress incontinence, while in six of these residual urine in the amount of 250 - 300 ml was determined after urination. 21 patients state one or two events of nocturia per night. The monitoring of the PSA decrease following radical prostatectomy

A prospective study monitoring the decrease of the PSA levels in relation to the volume of the prostate removed and to the duration of the surgical intervention. A correlation was found between a complication-free procedure and a quick PSA decrease down to zero levels. On the contrary a prolonged surgical procedure can be followed by a slow gradual decrease of the postoperative PSA levels. The prognostic value of this slow decrease will be explored during additional observations.
