Hora M, Stránský P, Ürge T, Dolejšová O, Svobodová H, Ferda J, Hes O. Laparoscopic nerve sparing extraperitoneal radical prostatectomy - video. Introduction: The most advantageous approach for radical prostatectomy has not been yet been proven based on evidence-based medicine. Great emphasis is placed on the erudition of the surgeon, whatever technique is used. However, there is a clear shift from open to miniinvasive technology. If the robotic system is available then it is preferred. Workplaces not equipped with this system must choose between open or laparoscopic approach. In our work we present our approach to laparoscopy. Material: 748 laparoscopic...

Pitra T, Pivovarčíková K, Sedláčková H, Hes O, Hora M, Daum O. Lynch syndrome in the hands of the urologist. Major statement: Review dealing with Lynch syndrome, its presentation, diagnosis, treatment and surveillance in the urinary tract. Lynch syndrome (LS) is an inherited autosomal dominant cancer syndrome. It presents in young/middle aged patients and it predisposes individuals to wide spectrum of malignancies in different organs (the most frequent are tumors of colon and endomemetrium). LS is typically associated with upper urinary tract urothelial carcinoma (UUTUC), which is considered to be the third most frequent tumor in LS. Recently, a large...

Fiala V, Sobotka R, Vaľová Z, Pešl M, Hradec T, Hanuš T, Čapoun O. Experience with the prostate health index in daily clinical practice. Aim: The use of prostate health index (PHI) for prostate cancer (PCa) diagnosis in patients with elevated prostate-specific antigen (PSA) level. Methods: Between 05/2015 and 05/2016 we performed 230 PHI examinations. We recorded age, PSA and free to total PSA ratio (f/tPSA) before the indication of PHI, PSA, f/tPSA and [-2]proPSA level measured in PHI calculation, size of the prostate, digital rectal examination finding and histology result in case of a prostate biopsy (PB). The distribution of parameters in...
Flow cytometry analysis of sperm quality in sub‑fertile men

Kratka Z, Luxova S, Vik V. Flow cytometry analysis of sperm quality in sub‑fertile men. Semen analysis does not provide enough information about sperm quality. Flow cytometry tests allow us to analyse percentage of apoptotic sperm, DNA fragmentation and acrosome integrity. Sub‑fertile patients are better diagnosed using these tests. Objective: Overview of sperm quality data measured by flow cytometry. Material and method: 800 sub‑fertile men (aged 21-66 years) were treated at GENNET - assisted reproduction centres. 366 patients had normal results and 434 patients had pathological parameters of semen analysis. Flow cytometry analyses...

Novák J, Fišer L, Jůzek R, Lesenský J, Pertlíček J, Špůrková Z, Beroušková P, Hyršl L. A solitary skeletal metastasis in the thumb of a silent bilateral clear cell renal cell carcinoma pT1a in a patient with a sporadic uro-oncological triplicity. We present a case report of a 72-year old male with a long urological history (prostate adenocarcinoma after radical prostatectomy with rising PSA level and urothelial carcinoma pTa low grade with one recurrence) who underwent reconstructive surgery for a left thumb metastasis of a clear cell renal cell carcinoma. Repeated CT scans showed two tiny lesions in both kidneys during follow-up. Nephron-sparing surgery...

Čečerle G, Šámal V, Mečl J. Paratesticular malignant mesothelioma. Paratesticular malignant mesothelioma is an extremely rare type of mesothelioma with limited number of published cases. The origin of this cancer is most often associated with exposure to asbestos with long latency between exposure and diagnosis. We present the case of an 80-years -old patient with hydrocele. Aspiration of this extremly large of the hydrocele was performed a few times prior surgery. We found large septated, haemorrhagic hydrocele with significantly thickened extranal layers of the testis, smaller testicle of solid consistency and atrophic epididymis. We indicated...

Čečerle G, Šámal V, Mečl J. Agranulocytosis after analgesic treatment for renal colic. Renal colic is a symptom characterized by cramp-like, recurrent flank pain, often accompanied by vomiting and other vegetative symptoms. Urologists encounter patients with renal colic almost daily and its diagnosis and treatment is one of the basic urological skills. We present a case of a 26-years-old patient who was examined with renal colic at our clinic. Renal CT scan showed the presence of 3mm stone in the juxtavesical part of the ureter on the left. The patient was instructed that the stone could pass spontaneously off the ureter and was sent home with analgesic...
Untypical secondary cancer after radiotherapy for prostate cancer

Tureček P, Sutorý M. Untypical secondary cancer after radiotherapy for prostate cancer. On two case studies from our department we present emergence of untypical secondary malignant disease at patients with a primary adenocarcinoma of prostate having been treated by conventional curative radiotherapy. We want to point out a general risk of secondary malignity, which can be omitted in practice, and certain diagnostic pitfalls as well.
