Aim: To present on video our current most used technique of robot-assisted resection of renal tumour (RR). Material: We performed 274 RRs between June 2020 and November 2024. Our technique is based on a modification of conventional laparoscopic renal resection, of which we performed 599 between August 2004 and May 2020. RRs currently account for over one third of the surgical procedures for kidney cancer at our institution. Laparoscopic (rarely robotic assisted) nephrectomy is almost as frequent. Open resection accounts for about 17% and open nephrectomy for slightly less. Open resections are mainly indicated for more complex tumours,...

Major statement: This systematic review article evaluates the efficacy of prophylactic mesh implantation in ileal conduit use, focusing on preventing parastomal hernias and analyzing the risks and benefits of this approach, offering a new perspective on preventing complications in stoma patients.Summary: Parastomal hernia (PH) is a significant complication in patients undergoing surgery with an ileal conduit, with reported incidence rates ranging widely from 10% to 65% in the literature. Due to the high risk and morbidity associated with PH, the potential for prevention through prophylactic mesh implantation is increasingly being explored....

In addition to hypogonadism, other endocrine diseases are associated with male sexual dysfunction. In this article, we review the role of the pituitary hormone prolactin in male sexual dysfunction, and in particular in erectile dysfunction. Erectile dysfunction is common in men with hyperprolactinemia. Treatment of this primary cause may affect normal erectile function. In our patient cohort, after treatment of hyperprolactinemia (medical and surgical), prolactin levels normalized in all treated patients. Erectile dysfunction in all men was treated with some type of phosphodiesterase 5 inhibitor - i.e. sildenafil, tadalafil, vardenafil, avanafil....

Introduction: Urine cytology is primarily used for the detection of high-grade urothelial carcinoma (HGUC) in a urine sample. The typical use of urine cytology is the detection of lower urinary tract UC. There is a relatively limited knowledge about the efficacy of urinary cytology in setting of upper urinary tract urothelial carcinoma (UUTUC), i.e., urothelial carcinoma of the renal pelvis and ureter. Material and methods: A total of 68 patients treated at the Department of urology Faculty hospital Pilsen were involved retrospectively (the time period 2017-2022). All patients had histologic diagnosis UUTUC (based on biopsy) and cytological examination...

Pure testicular teratomas are exceptionally rare among nonseminomatous germ cell tumors (NSGCT). Despite their limited chemosensitivity, upfront chemotherapy is recommended for metastatic patients due to the potential presence of another NSGCT subtype alongside teratoma in metastases. We present a case report of a patient with primary post-pubertal teratoma and retroperitoneal cystic metastases, along with negative serum tumor markers - an exceedingly rare clinical scenario. In such cases, cystic retroperitoneal metastases are highly likely to be formed by teratoma, making primary retroperitoneal lymph node dissection a preferred option over chemotherapy,...
Vasographic obliteration of a bleeding back muscle artery after bilateral nephrectomy of polycystic kidneys

It is often necessary to remove polycystic kidneys after successful kidney transplantation. The reason for this procedure is that their significant volume can oppress surrounding organs, including the transplanted kidney, which can lead to its dysfunction. Native nephrectomy is often performed bilaterally at the same time. It is advisable to know the risk factors associated with this operation. One of them is the reduced strength of the vascular wall ligaments, which can lead to a higher risk of bleeding both intraoperatively and postoperatively. We present the successful treatment of severe bleeding in the early postoperative period by an interventional...

Major statement: This article provides a comprehensive overview of the therapeutic options for generalized urothelial carcinoma. The course of treatment is demonstrated by a model case report. Recommendations and treatment effect are supported by recent clinical trial results and in accordance with the latest recommendations of the European Urological Association.Summary: The authors present a case report of a patient suffered from metastatic urothelial carcinoma and discuss the role and efficacy of pembrolizumab in the context of comprehensive genomic profiling of tumor tissue.
