Ces Urol 2007, 11(2):80-84 | DOI: 10.48095/cccu2007006
Aim: Prostate cell growth is dependent upon the level of active testosterone which, even at low levels, is a significant tumour promoter. Thus, monitoring the activity of the genes involved in the regulation of testosterone synthesis is of relevance for investigating the mechanism of prostate tumour cell origin and proliferation. The main goal of the present study was to determine the incidence rates of the most significant single nucleotide polymorphisms of genes of testosterone synthesis pathways in a population of Czech patients with prostate cancer. Methods: The study included a total of 237 patients with prostate cancer and...

Ces Urol 2007, 11(2):85-88 | DOI: 10.48095/cccu2007007
In study from January 2001 to December 2004 we prospectively performed 557 examinations in 356 patients with clinical suspicion for the presence of bladder tumor. All patiens underwent cystoscopy and sonography with transabdominal multifrequency probe (2-5 MHz) on 2102 Hawk (BK-Medical, Denmark) imaging system. All positive, suspicious or inconclusive endoscopic findings were histologically verified. Primary superficial urothelial carcinomas were diagnosed with sensitivity 85% and positive predictive value 100%. In reccurences sonography had sensitivity 52% and positive predictive value 90%. Sensitivity and positive predictive value of sonography in...

Ces Urol 2007, 11(2):89-92 | DOI: 10.48095/cccu2007008
Aim: The stapler is the standard of care for occlusion of hilar vessels during laparoscopic nephrectomy. This paper aims to evaluate the possibility of replacing the stapler by locking clips. Methods and Results: Between April 2005 and November 2005, a total of 35 hilar vascular occlusions during laparoscopy (24 radical nephrectomies for a tumour, 7 nephrectomies for a benign disease, and 4 nephroureterectomies) were performed. The procedures were done by three operators. Two operators with less experience in laparoscopy used a stapler in all 20 procedures. One more experienced operator carried out 15 hilar occlusions. In 12 (80...

Ces Urol 2007, 11(2):93-97 | DOI: 10.48095/cccu2007009
Objective: Von Hippel Lindau disease is autosomal dominant disorder (VHL) with prevalence 1/39000-1/50000. Its characteristic sign is tumours infliction of different organs. Kidneys infliction occurs at 59-63% patients, from which 24-45% forms are clear cell renal carcinoma (CRCC). Authors present three cases with clinical aspects. Methods: 1 240 patients were surgically treated in dept. of urology since January 1991 to November 2006, three patients (0.24%) were affected with VHL disease. Results: Case 1: Thirty seven-years old man was examined for instability, giddiness, hearing noise in the year 1992. He underwent...
LHRH analogue in the treatment of prostate cancer, EAU guidelines and the reality of everyday practice in Europe and Czech Republic

Ces Urol 2007, 11(2):98-102 | DOI: 10.48095/cccu2007010
High incidence of prostate cancer is a subject of continuous search for optimal therapeutic approach. EAU guidelines, based on the results of studies already being done, present generally recommended treatment modalities. But some partial changes derived from the results of ongoing global studies are admitted. Survey done among 150 european urologists has shown significant differences between guidelines recommendations and routine daily practice. Farther education seems to be necessary esp. in indication of LHRH analogs. The same survey given to Czech urologists similarly revealed an incorrect low use of LHRH analogs mainly in the therapy of the advanced...

Objective: To find optimal size of hydrogel implants and to standardize application methods in male with stress incontinence. Methods: In group of 12 male with stress urinary incontinence of different etiology swelling implants of two sizes made of 2-hydroxyethylmethakrylate were administered. Patients were divided into three groups according the method of implantation: 1. transperieal needle approach below the sphincter under endoscopic control, 2.through perineotomy into bulbar corpus spongiosum, 3. the same as method 2 with insertion of corpus spongiosum between corpora cavernosa. We evaluated subjective status, uroflowmetry, pad weight...

Ces Urol 2007, 11(2):112-114 | DOI: 10.48095/cccu2007012
The aim of the study is a retrospective evaluation of urological and sexual chronic sequelae in patients with a pelvic injury. The followed up group comprised 86 patients (33 women and 53 men) at the age between 16 and 89 years (women's range, 16-89 years; men's range, 17 - 87 years), the average age was 39 years (women - 41 years, men - 39 years). As compared to the fractures associated with a primary injury to the urogenital system, the incidence of urological and sexual disorders in severe pelvic injuries (Type B fractures with a diastasis of the symphysis exceeding 2.5 cm and Type C fractures according to the AO/ASIF classification) was higher...
Current approach to grading of the prostatic carcinoma: ISUP 2005 modification of Gleason system - review
O. Hes, M. Hora
Ces Urol 2007, 11(2):112-114 | DOI: 10.48095/cccu2007013
Modification of Gleason grading system for prostatic adenocarcinoma has been published in 2005 as so-called International Society of Urological Pathology consensus conference (ISUP) with following conclusions: Gleason score 1 +1 = 2 as exceedingly rare entity should not be diagnosed, especially in core biopsy. Most such cases are in fact non-neoplastic entities (adenosis). Gleason score 3-4. Such diagnosis is usually inaccurate, especially in core biopsy. There is usually poor correlation with prostatectomy grade and poor reproducibility among pathologists. Well-differentiated prostatic adenocarcinoma recognizable in core biopsy should...
