Significance of the current pathological classification of the urothelial lesions for the clinical practice

Ondřej Hes, Milan Hora

Ces Urol 2008, 12(1):5-10 | DOI: 10.48095/cccu2008002

Morphological classification of the urothelial tumors has been changed several times. Former simple and easy reproducible classifications have been replaced by more complex and detailed descriptive systems. Current urological/oncological practice use only limited information from pathologists. The most important informations with direct impact to the treatment strategy are basic histological type, stage and grade. Practical problems with interpretation of diagnoses of urothelial leasions according WHO 2004 classification are discussed.

The objective of this article is to present the literature with respect to the urogenital tuberculosis and complications, to demonstrate the broad and typical, but also atypical symptoms, course of the disease, the investigation methods including new modern tests and the treatment. It is important to draw attention to the fact that it is necessary to think of genitourinary tuberculosis during a differential diagnosis from the very beginning, especially always when we detect simultaneously sterile pyuria and acidic urine pH. Frequent paralel recurrent nonspecific urinary tract infections do not exclude the diagnosis of the genitourinary tuberculosis....

Palliative radionuclide therapy for painful bone metastases in patients with prostate cancer

Jiří Doležal

Ces Urol 2008, 12(1):21-30 | DOI: 10.48095/cccu2008004

Aim: To present results of the Samarium-153-ED-TMP systemic radionuclide treatment in patients with hormone-refractory prostate cancer and painful bone metastases.Material and methods: Thirty-eight men (aged 50-83, mean 70 years) with bone disseminated hormone-refractory prostate cancer and bone pain received 153Sa-marium-EDTMP. Mean applied dosage was 40 MBq per kg of patients body weight. Kar-nofsky performance status, pain score (Numerical Rating Scale), analgesic score (WHO) and blood count were evaluated before and 1 and 3 months after the treatment. Results:Significant pain relief was observed...

Ces Urol 2008, 12(1):31-36 | DOI: 10.48095/cccu2008005

Aim: We performed a retrospective analysis of pre-operative parameters in patients with prostate cancer (CaP) and assessed the risk of the locally advanced carcinoma after radical prostatectomy.Material and methods: Between years 2000 and 2006, 471 men with clinically organ confined CaP underwent radical prostatectomy (RP). All standard preoperative parameters were collected preoperatively Pathological stage and Gleason score (GS), as well as surgical margin status and perineural invasion in the surgical specimen were registered. An impact of the preoperative parameters on the pT stage was assessed by Spearman rank correlation...

New surgical method of treatment for male urinary incontinence - ARGUS adjustable sling implantation

Michael Urban, Jiří Heráček, Jan Hrbáček ml., Tomáš Novotný

Ces Urol 2008, 12(1):37-43 | DOI: 10.48095/cccu2008006

ARGUS sling is a new surgical method for treatment of urinary incontinence in men after prostate surgeries. We evaluate the efficacy and safety of a new adjustable bulbourethral sling. It is also simple and effective method for treatment of male urinary incontinence. The development of new biomaterials has advanced and this is a set designed specially for men. Preliminary results are encouraging with higher continence success rate, but also due to more exact measuring of "leak point pressure" with minimal obstructive symptoms.

Introduction: Primitive neuroectodermal tumour (PNET) occurs primaly as a tumour of gastro-intestinal tract. He rises from neuroectodermal and is classed into group of small-cells (round-cell) tumours. Primary PNET of kidney is very rare tumour, witch is occured rather in younger ill, average age 28 years (4-69 years). Tumour was represented either as extrasceletal Ewing sarcoma (ES), with which has coincident to many imunohistochemical and cytogenetic features. Therapy of PNET is different from renal carcinoma, therefore exact histopathologic differential diagnosis with ussing imunohistochemical methods is necessary. We present case report of...

Ces Urol 2008, 12(1):49-54 | DOI: 10.48095/cccu2008008

The spontaneus rupture of the urinary bladder is extremely rare. The relation with acohol abuse and radiotherapy of the pelvis is suspected. In a majority of the patients the cause of the rupture remains obscured. In our study we are presenting three pacients treated for spontaneous rupture of the bladder in our departement. Retrospectively we evaluated risk factors for spontaneous rupture and we try to find examination wich enable to establish this rare diagnosis earlier. Similary we show the treatment modalities accenting the possibilities of miniinvasive surgery.
