The essentials of physics of extra-corporeal shock wave lithotripsy

Kamil Fógel

Ces Urol 2010, 14(2):73-80 | DOI: 10.48095/cccu2010013

Extracorporeal Shock Wave Lithotripsy (ESWL) utilizes a specific acoustic characteristics of a shock wave, which enables fragmentation of urinary stone. Shock wave is a acoustic wave generated by the shock wave source with high energy and variable frequency. Stone disintegration is caused by interaction of the shock between two acoustically different mediums. The main mechanisms of shock wave action is spallation, squeezing, acustic cavitation and compression. This method was invented and introduced to the clinical practice in the early 1980s in Germany and from that time this method has earned its position as a first-line treatment option in nearly...

advanced methods of external-beam radiation therapy in the treatment of prostate cancer

Karel Odrážka, Martin Doležel, Jaroslav Vaňásek

Ces Urol 2010, 14(2):81-91 | DOI: 10.48095/cccu2010014

In the last 20 years, external-beam radiation therapy of prostate cancer made a remarkable progress - entirely new treatment methods emerged and treatment results improved. Treatment planning using computed tomography and calculation of the spatial dose distribution launched the development of conformal techniques of radiation therapy (three-dimensional conformal radiation therapy 3D-CRT, intensity-modulated radiation therapy IMRT). Subsequent progress in imaging technologies and a focus on accurate setup of the radiation beam have opened a period of image-guided radiation therapy (IGRT) that moved conformal methods on higher level of quality and safety....

Complications of treatment with bacillus calmette-guerin (BCG) in non-muscle-invasive bladder cancer

Ces Urol 2010, 14(2):92-98 | DOI: 10.48095/cccu2010015

Aim:Patients with non-invasive transitional cell carcinoma (TCC) a high risk of progresion may benefit from intravesical chemotherapy or immunotherapy Intravesical BCG instillation has better outcomes than Mitomycin instillation in patients with moderate to gross risk of disease progression. However this is accompanied by a higher degree of side effects.Material and methods:Together 56 patients were included into the study (42 males and 14 females). All of the patients had histologically proven non-muscle invasive bladder TCC with moderate or high risk of progression. BCG was initiated with six weekly doses followed by maintenance...

Urine survivin in noninvasive diagnosis of transitional cell carcinoma of the urinary bladder

Michael Pešl, Marko Babjuk, Viktor Soukup, Monika Szakácsová, Pavel Dundr, Tomáš Hanuš

Ces Urol 2010, 14(2):99-103 | DOI: 10.48095/cccu2010016

Aim:To assess performance of survivin level in urine for noninvasive detection of urinary bladder cancer.Materials and methods:There were 151 patients enrolled in this study (84 patients with urinary bladder tumoru and 67 screenings) from July 2004 to January 2006. Patiens were divided into different groups according to tumour stage, grade, multiplicity, size, new cases and recidives. Results:The mean survivin level in patients with tumour was 32.4 pg/ml, and 22.4 pg/ml in the screenings (p = 0.001). The mean survivin level of patients depending tumour stage, grade and size was significant. Cut-off of urine survivin...

Percutaneous radiofrequency ablation (RFA) in the management of renal tumours - own experience

Petr Moravek jr., Petr Moravek, Petr Dvořák, Petr Hoffmann, Jaroslav Pacovský, Pavel Navrátil

Ces Urol 2010, 14(2):104-109 | DOI: 10.48095/cccu2010017

Aim:Evaluation of clinical experience with percutaneous RFA of kidney tumours in the period 2006-2008 and indication for this method.Material and methods:Overall 36 kidney tumours in 33 patients were enrolled to the prospective study; 53 RFA operations were carried out. The operation was done by the Star-Burst instrument (RITA Medical system) under CT control under a local anaesthesia. Patients were admitted for a one day hospitalisation. Indications for RFA were serious comorbidities contraindicating open surgery, tumour in anatomically or functionally solitary kidney and tumours in chronicaly insuffitient kidneys.

Less common causes of dysuria in women

Andrea Kastnerová, Oto Köhler, Kamil Belej, Petr Hrabal

Ces Urol 2010, 14(2):110-114

We demonstrate two case reports of less common cause of dysuria in women. In the first case report the cause of patient's problem was leiomyoma in the urethrovaginal septum, which resulted in dyspareunia, pain in hypo-gastrium and intermittent micturition difficulties. We removed this pathological formation by open operation in general anaesthesia from section in vaginal introitus. In the second case report we found incrustated stitch in urinary bladder in woman investigated for urgency and recurrent urinary infections. The stitch was removed in general anaesthesia during endoscopy by the endoscopic forceps.
