Use of neuromodulation in the treatment of lower urinary tract dysfunction

Conservative treatment of lower urinary tract dysfunction includes behavioral modifications, pharmacotherapy and pelvic floor exercise. Pharmacotherapy is used most frequently, but it is not always effective and often associated-with adverse effects. If conservative treatment prooves unsucessfull, surgical methods including bladder augmentation and urinary diversions are used. Neuromodulation has the potential to fill the gap between pharmacological and surgical treatment. How safe is active surveillance strategy of small renal masses?

The changing aspect of the rising incidence of small renal masses (SRM) has led to the establishment of minimally invasive procedures with the intent to achieve equivalent or similar treatment results when compared to standard partial nephrectomy. Active surveillance (AS) in patients with SRM represents an alternative treatment option in high risk patients with short life expectancy or in those who refuse any kind of surgical intervention. The aim of this review is to present current knowledge and dilemmas on AS strategy.

Aim:Urological complications associated with indwelling urethral catheter in men after total knee and hip arthroplasty. An analysis focused on the incidence and risk factors leading to these complications.Methods:In 2008 and 2009, 94 knee replacements were peformed in 93 men, aged on average 69.9 years, and 124 hip replacements in 124 men, aged on average 74.5. Aim:The value of using fluorodeoxyglucose positron emission tomography (FDG-PET) before residual mass resection was studied in patients with nonseminomatous germ cell tumours (NSGCTs) after orchiectomy and platinum-based chemotherapy.Methods:Thirty patients with NSGCTs who had been investigated with FDG-PET in the preoperative period were evaluated retrospectively. We have calculated specificity, sensitivity, positive predictive value (PPV) and negative predictive value (NPV) of FDG-PET to correctly predict the presence of viable carcinoma, mature teratoma, and necrotic/scar tissue in postchemotherapy residual masses.

We present three patients, who underwent bilateral adrenalectomy for ACTH dependent Cushing's syndrome. Despite using all available examination methods, we failed to identify the source of ACTH overproduction prior to the time of intervention. Surgery was performed in our department of urology from 2009-2011 based on the indication of the endocrinologist. For patients undergoing bilateral adrenalectomies laparoscopic or robotic approaches have a clear benefit.
