Pharmacotherapy of castrate resistant prostate cancer (CRPC) has undergone remarkable changes within the last several decades. A crucial discovery of hormonal dependence of prostate cancer by Huggins and Hodges, in the late 1940s, opened new therapeutic possibilities. In the 80s, the U.S. Food and Drug Administration approved the first luteinizing hormone-releasing hormone (LHRH) agonist. These drugs have a suppressive effect on the testosterone levels in the blood, which is equivalent to an orchiectomy. Nevertheless, this method has been associated with negative side effects, such as miniflares. Also, the level of testosterone in the blood was not...

Aim:Radiotherapy has been one of the standard treatments for prostate cancer for decades. Rapid technological development over the last 20 years has changed the way we use radiotherapy to treat prostate cancer. Methods:This communication reviews study outcomes and summarizes the current knowledge regarding the use of newer technologies in the radiation treatment for localized prostate cancer. Results:Randomized studies have shown that higher doses of radiation improve clinical outcomes, but with increased toxicity to healthy tissue. The introduction of the techniques of conformal radiotherapy, intensity modulated...

Aim: We present a summary of our experiences and results with diagnostic algorithm and treatment of patients with renal injuries. Methods: A total of 172 patients with renal injury were treated at the Department of Urology in Pilsen between January 2005 and December 2012. We performed a retrospective analysis based on mechanism of injury, diagnostic algorithm used, significance of haematuria and choice of treatment. Iatrogenic injuries were excluded. Results: Non-penetrating renal trauma occurred in 170 patients (98.8%) and stab wounds in two cases (1.2%). According to the AAST (American Association...

Aim:To evaluate the feasibility and toxicity of dose escalation to the intraprostatic lesion to 84.84 Gy in 40 fractions using simultaneous integrated boost (SIB).Methods:Between September 2009 and September 2012, we treated 41 patients with intensity-modulated radiotherapy (IMRT) for prostate cancer using SIB. Intraprostatic lesion was defined by MRI or MRI plus spectroscopy transrectal ultrasound and physical examination. The prescribed doses were 84.84 Gy 80 Gy and 76 Gy in 40 fractions to the intraprostatic lesion, high risk volume and prostate with seminal vesicles (SIB 84.84). Late toxicity was prospectively scored according...

Aim:Urologists are aware of the advantages of using MRI for imaging renal tumors compared to computerized tomography (CT). To date, MRI biphasic angiography (MRA) has not been used for planning surgeries due to the low resolution of 1.5T machines. Recent technical improvements in 3T MRA enable shorter acquisition time and higher spatial resolution compared to 1.5T MRA. This allows for relatively high quality reconstruction of renal vessels. We present our experience using a 3T MRA.Methods: This study analyzed 155 patients with kidney tumors, who underwent 3T MRI (Magnetom SKYRA 3T, Siemens), between April 2011 and March 2013....
Is it appropriate to administer antibiotic prophylaxis to infants with severe hydronephrosis before pyeloplasty?

Ces Urol 2013, 17(3):193-198 | DOI: 10.48095/cccu2013027
Aim: The aim of this study was to assess the development of acute pyelonephritis (APN) in infants with hydronephrosis, detected using ultrasonography (USG), and severe uretero-pelvic junction obstruction requiring pyeloplasty. Based on the results, we evaluated the necessity for antibiotic prophylaxis until the time of surgery. Methods: A prospective study was performed to assess the presence of APN prior to pyeloplasty in infants with severe dilatation of the renal pelvi-calyceal system (PCS). The infants underwent standard USG and MAG3 scintigraphy examinations. In those with APN, a voiding cystourethrogram was performed....

Ces Urol 2013, 17(3):199-203 | DOI: 10.48095/cccu2013028
Methods:Over the course of one year we have performed 42 intraoperative biopsies and 14 computed tomography-guided percutaneous biopsies. In collaboration with the pathologist, we compared the biopsy results with the histological findings following nephrectomyResults:In intraoperative biopsies, the accurate histological diagnosis was achieved in 83% of patients compared to evaluation of the whole nephrectomy specimen. In CT-guided biopsies, the accurate diagnosis was obtained in 85%.Conclusion:This is a small sample size, but according to the results, we suggest that small renal mass biopsy may be beneficial.

Aim: The aim of this study was to evaluate the prognostic value of BCL-2 and BAX-1 gene expression in the tissue of non-muscle invasive superficial urothelial carcinoma. Methods: This study included 84 patients with Ta, T1 urothelial carcinoma of the bladder treated with transurethral resection. Mean follow-up was 23.64 months. Histopathological diagnosis of Ta was made in 48 patients, Tl in 36 patients, 23 patients were diagnosed with grade G1 tumor, 45 with grade G2 and 16 with grade G3. During the course of follow-up, tumor recurrence developed in 38 patients and progression was observed in 4 patients. The...
