Varicocele is rare in boys under 10 years of age, and becomes more frequent at the beginning of puberty. It is mostly asymptomatic and fertility problems arise in about 20% of adolescents. Venous reflux into the plexus pampiniformis is diagnosed using Doppler colour flow mapping in the supine and upright position. Clinical varicocele is palpable without the Valsalva maneuver (grade II) and is visible at a distance (grade III).The size of testis should be evaluated by palpation, orchidometry, but preferably by ultrasound. The surgical methods of choice are subinguinal or inguinal microsurgical (microscopic) repairs, or suprainguinal open or laparoscopic...

Aim:Standard treatment of lower T categories of penile cancer usually includes partial amputation. However, in a significant number of cases, this approach could prove to be an overtreatment. Today, we have a possibility to use other less, radical methods, which proved to be sufficient. This article provides a review of these treatment alternatives.Overview:An ideal method to treat a localized lesion would be its excision. Unfortunately, in penile cancer, this option is only rarely acceptable. A flat lesion type of PeIN (penile intraepitelial neoplasia) or carcinoma in situ can be treated by local therapy (5-fluorouracil,...

Radical prostatectomy and radiotherapy are main therapeutic options for patients with localized prostate cancer. Both modalities are associated with significant risk of complications and socio-economic consequences. In low risk prostate cancer patients, active surveillance represents an alternative therapeutic approach. In well selected patients, active surveillance can delay active treatment. The aim of this review is to objectively present data on inclusion criteria and summarize results of comparative studies in active surveillance.
Age specific PSA and PSA velocity - comparison on the basis of prostate biopsies

Ces Urol 2014, 18(3):216-224 | DOI: 10.48095/cccu2014029
Aim:To compare predictive value of age specific PSA (VSPSA) and PSA velocity (PSAV).Methods:Histology findings of benign prostatic hyper-plasia and prostate cancer were divided into groups according to number of biopsies performed. The data was evaluated separately for every order of biopsy in relation to age specific PSA and PSA velocity.1. Models of age specific PSA (ng/ml):MODEL A: 40-49 years: 2.5; 50-59 years:3.5; 60-69 years: 4.5; 70-79 years: 6.5MODEL B: 40-49 years: 2.5; 50-59 years: 3.5; 60+years: 4MODEL C: cut off value 3.5MODEL D: cut off value 2.52. PSA velocity (ng/ml/year): Using the method...

Aim:Modern magnetic resonance immaging (MRI) with 3 Tesla (T) magnetic field opens new possibilities in the diagnostics of prostate cancer. The aim of this study is to compare the results of 3 T MRI of the prostate with the results of transrectal ultrasound (TRUS)-guided biopsy of the prostate and to determine the role of 3 T MRI in the diagnostics of prostate cancer.Material and methods:In the period from 4/2011 to 4/2013 (1st period) 354 men were evaluated using 3T MRI of the prostate. One hundered ninety two of them (54.2%) underwent MRI before TRUS biopsy of the prostate (pacients with a clinical suspicion of prostate...

Ces Urol 2014, 18(3):234-239 | DOI: 10.48095/cccu2014035
Aim:One hundred seventy one patients were treated by extracorporal shock-wave lithotrypsy (ESWL) at the Department of Urology, Regional hospital Kolín, between years 2010 and 2013. The aims of this communication are to analyse the success and complication rates of the treatment using this method. In addition we compared efficiency of treatment in group of patiens with and without ureteral stent. Methods:Retrospective analysis of treatment outcomes in patiens, treated by ESWL in a period between 2010 and 2013.Results:The stone-free rate was achieved in 58.5% patiens. The ESWL without stent was...
Inflammatory pseudotumor of the urinary bladder presenting as a symptom of active vasculitis in a patient with granulomatosis with polyangiitis (Wegener's) - an unusual manifestation of ANCA-associated vasculitis

Granulomatosis with polyangiitis (GPA) is the most common type of ANCA-associated vasculitides (AAV) occurring in the Czech Republic. AAV are (predominantly) small-vessel vasculitides that can afflict any organ in the human body. Urogenital involvement is relatively rare and can cause diagnostic challenge. This is a case report of a female patient with GPA presenting with urinary bladder pseudotumour. Urinary bladder carcinoma was initially suspected. Its differentiation from localized inflammatory process caused by active vasculitis was very important as the treatment of GPA includes potentially toxic cyclophosphamide. Establishment of the right diagnosis...
