Introduction: The aim was to assess the presence of p53 and Ki 67 expression in patients with localized conventional (clear cell) renal cell carcinoma. Material and methods: Seventy patients with stage pT1 and pT2 conventional (clear cell) renal cell carcinoma were included in the study. Nephrectomy was performed in 58 patients and renal resection in 12. Expression of p53 a Ki 67 was detected in both cancerous and non-cancerous tissues of the kidney. Immunohistochemistry results were evaluated semiquantitatively using HSCORE. During a mean follow-up of 21 months tumour recurrence occurred in 8 patients. Results: Expression...

The indication of radical nephrectomy in renal cell carcinoma clinical stages III and IV

J. Pernička

Ces Urol 2006, 10(1):10-14

A retrospective study compared cohorts of patients with stage III and IV renal adenocarcinoma treated at the Clinic of Urology between 1988 and 1999. 122 patients who underwent surgery and 65 patients treated conservatively were evaluated. By comparing the adjusted survival of stage III patients with or without radical nephrectomy, a survival 49 months longer was found in those who had undergone surgery; however, the difference was not statistically significant (p = 0.200). The same comparison was made for stage IV patients which showed a survival of 18.1 months vs. 14.6 months in favour of those undergoing radical nephrectomy. No statistical significance...

New laboratory markers in bladder cancer work up

R. Fiala, D. Stejskal

Ces Urol 2006, 10(1):15-18 | DOI: 10.48095/cccu2006004

Purpose: Endostatin and midkine are proteins which directly affect angiogenesis and are likely to be involved in tumour differentiation. E-FABP is an epithelial protein likely to be released when malignant lesions arise. The aim was to assess the options for determining both serum and urine concentrations of endostatin, midkine, and E-FABP as tumour markers in individuals with bladder cancer. Material and methods: The study enrolled 96 people, including 64 people with endoscopically and histologically confirmed bladder cancer and 32 people with benign urinary tract disease. All underwent cystoscopy (histology) and venous blood and urine test...

The results of the study Kapros in the region of Olomouc

V. ©tudent, R. Fiala

Ces Urol 2006, 10(1):19-22 | DOI: 10.48095/cccu2006005

Results: There were 2,585 probands. A total of 85 GPs were involved who examined 1,370 (53%) men. Further, 32 urologists from the Olomouc region participated in the study. They examined 1,215 (47%) males. In 101 of them, prostate biopsy was indicated and performed. Prostate cancer was detected in 24 (4.8%) cases. Smokers and obese males had a significantly lower level of PSA (by 0.1 ng/ml).

Ces Urol 2006, 10(1):23-26 | DOI: 10.48095/cccu2006006

Objective: At present, we are witnessing a boom in reconstructive urology. Urinary diversion is indicated not only in patients with locally advanced bladder malignancy but also in patients who have failed conservative management of lower urinary tract dysfunctions. Heterotopic neobladder is an ideal solution for patients who are not suitable candidates for orthotopic bladder replacement. A major challenge in heterotopic neobladders is the creation of the continent mechanism. The authors reviewed their experience with a heterotopic pouch which was described by professor Yachia from Hadera town (Israel) and named after his hometown. Material...

J. Krhut, P. Hradílek, K. Mainer, O. Zapletalová

Ces Urol 2006, 10(1):27-31 | DOI: 10.48095/cccu2006007

Introduction: Multiple sclerosis (MS) is a systemic autoimmune disorder affecting the structures of the central nervous system (CNS). In a high proportion of cases it is also manifested by symptoms of lower urinary tract dysfunction (frequency, urgency, urgent incontinence, evacuation dysfunction). By contrast, impaired function of the upper urinary tract is reported rarely. Our aim was to analyse upper urinary tract function in our own cohort of MS patients. Material and methods: The authors present an analysis of upper urinary tract function in a total of 148 multiple sclerosis patients (39 men, 109 women). The mean duration...

Ces Urol 2006, 10(1):32-39 | DOI: 10.48095/cccu2006008

Objective: Laparoscopic resection (LR) of a renal tumour is technically more demanding and has some limitations when compared to open resection (OR). We present our experience with introducing the method and compare LR and OR methodologies. Methods: From September 2004 to February 2006, a total of 179 renal tumours were treated surgically. In 58 (32.4 %) resection was performed. In 21 of these laparoscopic approach was indicated (36.2 %). We perform LR using hilar clamping followed by resection with scissors and suture of the kidney base and margins with an intracorporeal stitch. In one patient, conversion was performed because...

Ces Urol 2006, 10(1):40-43 | DOI: 10.48095/cccu2006009

Stress incontinence is one of the most important things, that evaluate negative quality of life millions of women all over the word. Prevalence of stress urinary incontinence is in adult women, but it occcurs in all women population. Only a small part of them purpose these problems to solve. Stress urinary incontinence is defined as any involuntary leakage of urin. Presently it is evolved a big effort accros the world to health education and awareness in female urinary incontinence problem. Treatment of stress incontinence became to be more successful with using tension free tape applications. The next important step in the treatment of stress urinary...

Abnormal course of fibromuscular dysplasia of renal artery

P. Drlík, O. Kóhler, J. Kočárek

Ces Urol 2006, 10(1):44-47 | DOI: 10.48095/cccu2006010

Fibromuscular dysplasia is a congenital abnormality of arteries. We present our experience with acute renal artery thrombosis caused by fibromuscular dysplasia in a young man. This patient was sent to our department from a county hospital where he had been treated for 14 days. His symptoms were very unspecific and resembled acute renal colic. On Doppler sonography revealed arterial flow only in the upper part of the left kidney. On DSA, fibromuscular dysplasia with stenosis and thrombus in the left renal artery were described. Functioning arteries were found only in the upper part of the kidney. We discuss the importance of DSA and Doppler sonography....

Sulcus nervi dorsalis penis/clitoridis: a novel marker of gender dimorphism of pubis

J. ©edý, O. Naňka, J. M. Walro, J. ©pačková, L. Jarolím

Ces Urol 2006, 10(1):48-54 | DOI: 10.48095/cccu2006011

The aims of this study were to describe the anatomy of a distinct sulcus on the caudal and ventral surface of the pubis, and to assess its relevance as a surgical landmark and as a feature that could be used to determine the gender in isolated pubic bones. We examined 6 male and 6 female formalin-fixed cadavers (age 52-86 years) to establish the position of the sulcus relative to soft tissue landmarks. Several parameters of the pubis and sulcus were measured in 286 isolated pelvises and analyzed by stepwise discriminant function analysis. In males, the dorsal nerve of the penis coursed in the sulcus whereas the dorsal nerve together with the dorsal...

Possibilities of using videourodynamics in VLPP detection (valsalva leak point pressure)

R. Vrtal, F. Zá»ura, R. Fiala, A. Vidlář, M. Grepl

Ces Urol 2006, 10(1):55-58 | DOI: 10.48095/cccu2006012

This work evaluates the contribution of some new methods of investigation of the lower urinary tract function, including the critical evaluation of the new original options of diagnostic procedures. It assesses the validity of videourodynamic VLPP detection by comparison of three urodynamic potentials (evaluation of uroflowmetry VLPP detection, evaluation of visual VLPP detection, and evaluation of video VLPP detection using an external camera connected simultaneously to a urodynamic device). Valsalva leak point pressure is presently the most important test for urodynamic detection of female stress incontinence. The principle of this is the most accurate...

A. Petřík, E. Alterová, M. Fiala, J. Novák, F. Zá»ura

Ces Urol 2006, 10(1):59-63 | DOI: 10.48095/cccu2006013

Concrement fragmentation in the ureter using ESWL is influenced by the type and properties of the device used, particularly including the Shockwave type and the way of targeting the concrement, the total energy used during therapy, and the Shockwave frequency. Of major importance is also the strucuture of the concrement as well as the presence of a stent. In a retrospective study we evaluated the influence of stent placement in treating ureterolithiasis using extracorporeal lithotripsy. In cases with placement of double pigtail stent, a higher retreatment rate of 1.92 was found as compared to that of 1.56 with in situ therapy (p = < 0.0001) suggesting...
