The coincidence of urothelial carcinoma of upper urinary tract and the bladder

The authors present a cohort of patients with a tumor of upper urinary tract in whom the coincidence of bladder urothelial carcinoma was surveyed. 48 patients with urothelial carcinoma of upper urinary tract were diagnosed and underwent surgery from 1988 to 1998. Patients were followed up for 14 to 144 months after surgery. The coincidence of carcinoma of upper urinary tract and urinary bladder was observed in 44% (21 patients). When comparing the grading of upper urinary tract tumours with bladder tumours, in 52% it was the same, in 38% higher and in 10% lower. The authors give a report of their active approach to patients suffering from a serious affection of the urinary tract caused by tumours of gynaecological or colorectal origin. Small cell carcinoma of urinary bladder is a rare disease with poor prognosis. About 150 cases are reported in the literature. The tumour is characterized by an aggressive growth with local spreading and remote metastases. The invasion of the tumour to the muscle of the bladder or to paravesical fat is present in most patients at the time of diagnosis and the incidence of metastases is high as well. The authors compared the effectiveness of standard diagnostic procedure and 3D reconstruction of ultrasonography in a group of 22 patients. The ultrasonography was independently evaluated from the record by two physicians - an expert and a sonographist in training. The aim was the differentiation between localized and locally advanced carcinoma of the prostate. The criterion was the histological examination of the prostate after radical prostatectomy. The authors report of a technical innovation destined for routine investigation of urodynamic methods, originating from the observation of urine leak from urethra. They used the videourodynamic inlet of the apparatus and they connected it with standard endoscopic optical device with a cammera, which enables a very precise scanning of urethral ostium synchronically with pressure curve of intravesical and intrabdominal pressures and EMG. The leak of urine can be analysed from the recording of the urodynamic protocol.

The authors used the technology of multi-channel static perfusion urethral profilometry with the aim to compare pressure values in individual channels and to find the information value of one channel only to evaluate the validity of standard one-channel static profilometry. They investigated a group of 25 patients with the method of four-channel static profilometry. The group consisted of patients with incontinence, BHP, urethral stricture, after RAPE and TUR. They proved, that the pressures in individual channels are so different, that the standard method of static profilometry is not reproducible and renders dianostic information of poor validity....
